We’re happy to be a part of the Pennsylvania Consolidated Waiver program, which offers a range of home and community habilitation support services to those with intellectual and developmental disabilities. What Is the Pennsylvania Consolidated Waiver Program?

The Pennsylvania Consolidated Waiver is a federally funded program that helps individuals with an intellectual disability, autism, or a developmental disability live independently in their homes. Waiver services are administered through partnerships with community-based agencies like Liberty Resources Home Choices. Program participants can access a variety of personal care, behavioral support, and community services.

Who Qualifies for the Waiver?

To qualify, you must have been diagnosed with an intellectual disability, autism, or developmental disability before age 22. You must also demonstrate substantial adaptive skill deficits in at least three areas of major life activity, which are:

  • Self-care
  • Language use and comprehension
  • Learning
  • Mobility
  • Capacity to live independently

Home and Community Habilitation Services

As part of the Pennsylvania Consolidated Waiver program, Liberty Resources Home Choices can provide an array of services to participating community members, helping them to improve their self-help and socializing capabilities. We offer the following services:

Behavioral Support

This service starts with a comprehensive assessment and the development of a behavioral support plan. This plan includes support strategies for the individual and potential interventions and training for the recipient and their caregivers. Behavioral support may be more or less intensive, depending on individual needs. This service is available for those ages 21 and older.

Chores and Homemaking

Much like the homemaking services offered by Liberty Resources Home Choices, chore services through the waiver program consists of helping to maintain a clean and safe home. Chore services go beyond homemaking by helping qualified community members with more intensive activities, such as:

  • Washing floors, windows, and walls
  • Securing loose rugs and tiles as safety precautions
  • Moving furniture to prevent falls
  • Ice, snow, and leaf removal

If a program participant rents a property, the landlord will be notified of the ongoing program, and they will still be responsible for all property maintenance according to local regulations.

Community Participation

This service provides opportunities and support for participating members. It helps them build community inclusion and connection, participate in social groups, and develop skills for potential employment.

Companion Services

Intended to assist individuals ages 18 and older, Liberty Resources Home Choices helps participants create a more meaningful and integrated home and community life. The companion program through the consolidated waiver is similar to the lifestyle support we offer any person with a disability.

Respite Care

The waiver program allows us to provide short-term emergency and 24-hour respite care. This service is offered to the direct care worker of waiver participants who need relief or have a temporary planned or unplanned absence.

There may be other services that qualified Pennsylvania Consolidated Waiver program participants can take advantage of, which enables them to maintain an independent and fulfilling lifestyle.
