How to Enroll

If you have been approved for waiver services and would like to choose Liberty Resources Home Choices as your Home Care Agency, call (215) 634-2000 ext. 617.

 

If you would like to receive waiver services, and have not yet been approved, you need to contact the Pennsylvania Office of Long Term Living.  Below is the information on how to apply. 

 

For individuals between ages 18-59

If you would like to enroll for waiver services you may do so by calling the Pennsylvania Independent Enrollment Broker named Maximus.

Maximus completes an assessment of your needs and determines your eligibility.  Following the assessment, you will choose a supports coordination agency.  If you are eligible for waiver services and decide to choose Liberty Resources Home Choices as your home care agency, ask your supports coordinator to send Liberty Resources Home Choices referral.

For individuals ages 60 and over

Most older Pennsylvanians prefer to remain in their home and stay connected with their community as they age.  Home and community based care provides an older person with the programs and services to help them maintain their independence.   Contact your local Area Agency on Aging then click on your County of residence to learn more about these programs.   

 

In order to qualify for Aging Waiver services, you must meet eligibility requirements. There is no cap on the services that Aging Waiver participants receive, no cost-sharing and no contributions or co-payments.  If not financially eligible for waiver services, an individual may choose to receive services through the state Options Program. These services are provided on a sliding fee scale, according to ability to pay.  Eligibility for services is based both on the individual’s need for care, and on ability to pay for services. There are formulas that take into consideration income, assets and expenses, to determine on a case-by-case basis whether a person is eligible to receive services at no cost; on a sliding scale; or on a personal pay basis.  Additional services are available through the Family Caregiver Support Program.
